Supported living is used to describe the arrangement whereby someone who already has ,or who wants their own tenancy or own home, also has support from a “Care and Support” provider to help them live as independently and safely as possible. They will receive support and help with any aspects that are required to live an ordinary life as possible, this could be help with Managing Bills and Money, Cooking, Personal Care, Managing Medication and with Jobs. People who live in supported living arrangements can live in a variety of different settings like with another person or others that they have chosen to live with in shared accommodation e.g. a shared house in which they all contribute to the bills and upkeep of communal areas. Or on their own in their House/flat/Bungalow etc.

TRANSITION CARE

One of our main areas of support is transitional care. This is for people coming from home in to supported living for the first time or from children services in to adults and felling quite overwhelmed and anxious.

For any person moving in to a new home it can be a daunting experience let alone for those having their first experience in a care setting. That is why we provide transitional services that are bespoke for every individual. We can arrange meet and greet visits to the homes and meet service users and staff. We can provide enabling support as a starting point to help build your confidence.

We as a service find transition is key to our service users feeling settled happy and confident. So we aim to make it as person centred and consistent for each individual as possible.
